You can register with us as an NHS patient if you live within our practice area. You will be registered with the practice, but you have the right to express a preference to see any of the doctors. Please make this clear when you register.

Please come to the surgery during opening hours (8:30am - 6:30pm). You will need to complete a form with details about yourself and your previous GP. It is very helpful if you have your NHS card or NHS number. You can also complete the form on behalf of other members of your family.

Please let us know if your name, address or phone number changes so that we can keep your records up to date. It is also helpful to have mobile and work phone numbers where possible.

There are a number of patients who are registered at Boundaries Surgery who have moved outside the practice area. While this does not present any problems for routine appointments at the surgery, it is a problem when a home visit is required or out of hours care is needed. It has also come to our attention that some patients are using the address and phone number of a relative who live in the practice area. This can cause considerable problems when we need to contact you and it also has the potential to cause problems regarding issues of confidentiality. We strongly recommend that patients are registered with a GP in the area that they live. If you have any queries regarding this please feel free to discuss this with a member of staff.
